Information we collect

Contact and booking information such as your name, phone number, email, appointment address and the services you choose.
Health information you provide during intake and your provider review, such as medical history, medications, allergies and symptoms, and records of the treatment you receive.
Payment information, which is handled by a third-party payment processor. We do not store full card numbers on this website.
Website information such as basic, anonymous usage data collected by our hosting provider. No analytics or ad tracking is currently installed on this site.

How we use it

To schedule and deliver your appointment and communicate with you about it.
To allow a medical provider to review your health history and authorize treatment, and to keep treatment records.
To process payments and, if you ask, provide receipts for HSA/FSA reimbursement.
To comply with legal and professional obligations.

Third-party services

Online booking and intake are handled through IntakeQ, a third-party scheduling and intake platform; intake forms are handled through IntakeQ, a HIPAA-compliant platform, and information you enter there is also subject to IntakeQ’s privacy practices.

Health information

We treat the health information you share with us as confidential. It is used only to provide and document your care, and it is accessed only by the people involved in your treatment and the vendors who help us deliver it.

Sharing

We do not sell your personal information. We share it only with the providers and vendors who help us deliver your care (such as our reviewing medical provider, booking and payment platforms), when you authorize it, or when required by law.

Text messages

If you text us or provide your mobile number, we may text you about your appointment. Message and data rates may apply. Reply STOP to opt out of non-essential messages.

Security & retention

We use reasonable safeguards to protect your information. Treatment records are kept as required by New Mexico law.
